This is the list i got. Maybe 1 or 2 items are arguable, but i leave it up to you to match your needs. With the world as it is today, a perfect country list just cannot exist 
The important thing is that 99% of the items in this list are countries or autonomous political entities.
Concerning the american states i don't know...i am not american either, but they don't change their states names and there aren't that many, so imagine that list you have is complete.
