Hi I experience the same problem but with J1.0.15 and CB 1.2 RC2.

I guess your issue is different. Maybe a directory write permissions problem for images/comprofiler ? I thought that was the issue.
I gave full control permission to users and IIS_WPG for images/comprofiler and all child objects. It seems like only Thumbnail will inherit the user groups and permission, and the profile image itself won't.

I'm still trying figure out what the issue is. I gave a write permission for IIS_WPG and users for images/comprofiler, but it's still not working. Can someone please help?
I'm running Joomla 1.0.15 on IIS 6.0 with PHP fast cgi
