So let me get this straight, all lower case for the headers, and do I need the cb_ in cb_uniquename?

safest thing to do is an export first to see the field names. That usually explains the logic to most people. The important point is that at least one cb field must be in the edit/add run. In my example, I used middlename rather than one of the user specified fields which include the cb_

No. Order is independent. You do not need to use all the fields just the minimum set. See my example. The middle name field serves as the required CB field.

Some people have had problems with NON-English web sites and this component. If that is your case, be sure to check the character set of the tables etc. You can check that with phpmyadmin.
