Profile gallery is misbehaving!

I've got some really strange behaviors right now with Profile Gallery.
1. Some images are duplicated when uploaded.
2. Some images upload and create a thumb, but the tables aren't written to the DB so the images don't show up. The "count" of images shows the total images, say 12, but I may only see 10 images in the gallery. I can see the files on the server.
First, I'm running what I think is Version 1.0 RC2++ of Profile Gallery.
I found a version here called 1.0RC2SR1+ so I figured I needed to update it and maybe that would solve the problem. But when I unzipped, the folder it unzips to is 1.0RC2++! So is there only one version or how does one tell?
My system:
Database Version: 5.0.45
PHP Version: 5.2.6
Web Server: Apache/2.2.9 (Fedora)
WebServer to PHP interface: apache2handler

I still have the double photo upload problem. I have an update though. This happens on Firefox 3.05 for Mac. I just tried uploading images in profile gallery on Safari and it didn't double up the photos.
