Is the second image a image field? If it is then they just update it from profile edit like any other image field. You mentioned adding an image to the description. That doesn't add a field or anything, that's just adding an image to display. You can add more image fields within CB > Field Management.

Different default image? Not without using the delimiter field trick explained in my previous replies. In CB 2.0 it's just a matter of changing a parameter in the image field though, but it's not so easy for CB 1.x.
