Is there a way to force profile edit (or at least one field -- accept terms) to be completed on subscription renewal?

There's no specific feature for that in CBSubs so the only way would be to use something like CB Auto Actions to redirect them to profile edit when they try to renew. For terms and conditions however you can enforce that they review and accept the terms separate from subscription though. To do this edit CBs system plugin "Community Builder System plugin" within Extensions > Plugins and enable "Verify Consent".

Would this happen at each renewal or only when they originally sign up?

The verify consent is enforced as long as they're logged in. They won't be able to renew, subscribe, or anything else until they accept the terms or for example delete their account via Delete Me field from CB Privacy. The verify consent also works with as many terms and conditions field as you like the only requirement is that they be marked required and accessible to the user to edit.
