At this time SQL Actions fires on First Activation, Renewals, and Upgrades all at once. They're not individually configurable as with a few other integrations, but this is to be fixed in a later release. 
For E-Mails please see the CBSubs Email integration plugin followed by configuring the Activation Email. This email will only send on first time activation or upgrading to the configured plan.
