Have reviewed your install. Your issue is caused by your Joomla template. It's converting all select elements into jQuery Chosen usages. None of our plugins are designed to handle jQuery Chosen elements and are designed for standard select elements. Switching to default Joomla protostar resolves the issue. You'll need to see how to disable that feature in your Joomla template or at least a way to exclude CB from it.
