MySQL Insert errors

I'm getting an error on each installation attempt, and have even tried using phpmyadmin to look for duplicate entries or other problems, but I see nothing wrong in the database.

Any ideas what would be causing this? Joomla 1.0.8, all permissions are set correctly.

When I try to uninstall it tells me it failed. I have to manually remove every file and each database entry. I really don't think I'm missing anything, I looked at every table. I don't see any entry anywhere that this error is talking about.
Also, this happened on the first attempt, on the clean installation with a new database.

Sadly that will take me a few hours, with reinstalling the templates and reconfiguration.
But I'm not sure I have a better solution, if it happens again I'm going to be very upset...
