[SOLVED]Registration Fields Missing

I desperately need help. When a new user attempts to register at AutismMAPS.com, there are no fields for Username, Name, Password or Email. They have disappeared! Please set me off in the right direction - thanks!

make sure your cbcore plugin is set to public and that your contact info tab to everyone.
