What happens is the user is active in a group, but they don't actually have access to that group. This can happen if you forced the user in and they're Registered, but the group is Author for example. This was exactly your case as you have the group set for "Manager" and the users that kept showing up were "Registered".

It'll now save regardless, but each notification has its own individual permission so it prevents them form receiving notifications they've no permission to receive.

Thanks for the fix.

we're testing a GJ2.5 installation with CB1.8 and J1.5.25. On the Tools menu we had many (hundreds) users listed as "Missing Notification", then clicked Fix All and most of them went away, but still have a dozen or so that do not go away when clicking Fix All. Can you explain what this means?

you said:

It simply checks if a user has a row in the notifications database and if they should have a row then adds them if they don't exist.

but I'm not sure what this database is.

PS - the entire list looks like this (and Fix All does nothing for the Group owner list and the User list):
:: Group :: Missing owner notifications - 12, 23, 20, 19, 18, 6, 14 - Fix All
:: User :: Group does not exist in GroupJive -
:: User :: Missing notifications - 580, 520, 498, 488, 361, 344, 179, 174, 157, 537, 355, 181, 538, 511, 508, 502, 346, 345 - Fix All

we're testing a GJ2.5 installation

GJ 2.5.1 is released, please upgrade. This has been fixed already and notifications will save regardless of category/group access permission.
