Image not displaying on any page:
Site URL: marcellomonteiro.com/index.php?option=com_acctexp&task=subscribe&usage=3
CB Captcha Image URL: marcellomonteiro.com/index2.php?option=com_comprofiler&task=tabclass&Itemid=1&tab=getcaptchatab&no_html=1

Joomla! Version: 1.5.2
CB Version: 1.1
CB Captcha Version: 1.0

gd
GD Support enabled
GD Version bundled (2.0.28 compatible)
FreeType Support enabled
FreeType Linkage with freetype

No SMF
No Joom!fish
Not the Template (tried both included with Joomla!)
No Extra Spaces in default language file/English

Shouldn't be a server problem because CB Captcha works on other sites on the server.

Quoting myself seems odd but anyways....

I ended up installing Captcha 2.0.1 and my image still does not show up. It doesn't show up in the back end either.

Plugin Prerequisites
Requirements: GD: bundled (2.0.28 compatible)
Freetype: enabled

To show up the image with security code you only need to set session handler to None in joomla system configuration. I think it's set to Database by default. I change it to None and that's helped me.

I am having the same issue. The security image is not showing up neither in the back-end nor in the frontend. Here is my setup:

Joomla 1.0.13
CB 1.1
Captach Plugin 2.0

I have install the cb.captcha but I can't see the image on the front page. The url from tha image is the following.

www.xxx.com/index2.php?option=com_comprofiler&task=tabclass&Itemid=1&tab=getcaptchatab&no_html=1

I have already install the GD Gallery and the configuration of PHP file is the right as you have mentioned.

What can I see for checking that?
