Adding New Form Field Types

Adding New Form Field Types was created by jpickel
We have a need to add in a dymanic <select> drop down field. We would like to have 3 <select> boxes, but the content for the final two would be filled in, depending on the <option> selected in the first field.

It would function like this:
javascript.internet.com/forms/dropdown-box-population.html

Does anyone know of a plugin or component to make this possible?
