Is the directory shown in the error message correct for phpbb?
/home/****/public_html/forum/extension.inc

The error is indicating that it can not find the file extension.inc, and then doesn't know the extension so it can not find the phpbb common.php file... so it also can not find the phpbb encode_ip() function...
