I assume that name comes from a field

what i'd like explained is how to get those fields, display them and how to create them

I know it's a bit noobish stuff, but as mikko already remarked:

This seems to be a problem that many peopel are asking here.

I think that when I know how to make a plugin completely (including fields), then I'd have loads of plugins submitted here every week . This really really helps understanding plugins more and more.
