Taking one at a time:

• Did not upgrade from CB 1.x, clean CB 2.0 install
• Disabled Joomla SEF Plugin - no change
• Have published Joomla CB profile entry, visible after login
• Switched to default Protostar template - no change
• Enabled debug mode in Joomla, get some error messages about Invalid CSS but nothing else significant

I think it maybe significant to note that the subscriptions do work when going through a new user setup if I enable Allow Registration, but not during an existing user Upgrade.

Totally out of ideas what to try next.
