Try comparing your old promotion to your new one to see what configuration is different as it could just be a configuration problem in your old promotion.

I did this and saw nothing different. I created the now non-working promotions about 6 months ago and didn't have a problem with them. I did not make any changes to the promotions, only updated CB and CBsubs when new updates were available. My promotions that stopped working used mainly default settings.

Strange, I unfortunately don't have any explanation for that behavior. Since the new promotion you created is working fine I suggest just continuing to use the new one. If the issue happens again please let us know as we'll need to completely debug the install process for CBSubs Promotions to see if there could be a migration issue or something of the sort.
