Here is what I have set now and it does not send the confirmation email.
Show subscription plans at registration: Yes
Allow Free Registered Users (without subscriptions): No
Create Subscriptions also for free lifetime plans: Yes

I also tried this and it does not sent the confirmation email.

This issue has caused us to completely come to a halt and we are on a tight deadline.

Any more ideas?

User can choose membership with 1 of 14 options and then can choose 1 or 2 merchandise items and a donation item.

I too am using all non-exclusive plans. Is no email address working or is it just 1 email address you're trying?

Have you checked your server or php error logs to see if there was an error?

Have you examined console with Web Developer or Firebug for Firefox for any JS errors?
