Plugin: CB Paid Subscriptions
Plan: PLAN_ID_HERE
You can find the plan id under the far right id column within CBSubs > Plans. If that's not working or available you can create an External menu item using the URL provided at the top of your plan while editing your plan in CBSubs > Plans (begin with index.php).

Yup, create a Joomla menu item with Type set to Community Builder > Plugin. Next configure the following.Is there a way to give each subscription plan its own menu item?
Plugin: CB Paid Subscriptions
Plan: PLAN_ID_HERE
