Is there a way to add a subscription and basket by hand. I can go into the member and click on the new plan, thats easy. I have a client with a plan that has a parent plan of a upgrade to full membership +10.00

So for instance the person is a associate plan 45.00. parent plan upgrade to full 10.00, this one CB user is so screwed up that I deleted everything, like she was brand new. And then under her membership add the associate-parent plan upgrade..10.00.

But no basket was made so we cannot pay it offline.

How do I create a basket by hand?

Login as super user on frontend, navigate to the users profile, go through the subscription process as normally using the subscription tab on their profile. Once you're done creating the offline basket and clicking for the payment slip you can login to backend and pay the basket. Currently this is the only way to create a full basket and payment process on a users behalf.
