I just discovered that the issue gets resolved when I disable the Regular Labs - Email Protector plugin. This is a plugin from Regular Labs that cloaks email addresses on the site. Now I am not sure if something changed because of the upgrade to CB 2.1.2 or an upgrade by Regular Labs (I am contacting them as well) because it was working fine before.

No idea what their extension is doing. I'm guessing just some faulty jQuery is breaking JS further down the chain resulting in the ajax response not working properly. They also shouldn't be loading any HTML or doing any sort of processing on &format=raw or &format=json as they're going to break ajax endpoints doing so.
