CB 1.9 / Bootstrap plugin enabled
When I try to upload a new profile image i get this error Error: 500 - JHtml: :bootstrap not supported. File not found.
Upon returning to user profile, the image is actually there, but the error is confusing. (screenshot after clicking upload image attached)
Debugging mode on reveals:
Notice: ob_end_clean() [ref.outcontrol]: failed to delete buffer. No buffer to delete in <span>/</span>home<span>/</span>throwd5/public_html/wodrocket.com/pivot/plugins/system/debug/debug.php on line 87
Not Found
The requested URL /pivot/ref.outcontrol was not found on this server.
Additionally, a 404 Not Found error was encountered while trying to use an ErrorDocument to handle the request.

As I understand this is due to some 3rd party Joomla templates not properly loading in Bootstrap. Please test with default Joomla template and see if issue persists.
Done some further searching and looks like Gantry can cause this. Please see the below.

I simply downgraded Gantry and it worked fine.
