Those are actually just checkbox fields set to display as Yes/No buttons. You can make such fields in CB > Field Management. Once done you can map them to notifications in CB Activity > Parameters > Notifications and you're all set with per-user notifications.

Those in particular though were created automatically with CB Quickstart among many other fields. Feel free to login to the backend of the demo site using the Administrator button under Login As. There you can take a look at CB Activity > Parameters > Notifications and those fields in CB > Field Management for a good example.
