This is the only approval I have on the site - the avatar is not approved after the error, it fails.

I think your issue is coming from AcyMailing. I've no idea why through. Looks like it hooks into and overrides Joomla mailer? Unpublishing "AcyMailing - Override Joomla emails" resolves the issue. Was able to approve the image fine. Note I have published it again after quickly testing this. Suggest contacting AcyMailing as seams like a possible bug.
