CB Captcha fails to install

CB Captcha fails to install was created by jr39
I am working with Joomla 1.5.14, CB 1.2.2 and trying to install the current CB Captcha plugin using the CB Plugin Manager. Initially I simply got a blank screen. After turning on debug and most detailed error processing, the following message appears:
