There is no functionality to add more fields to the moderate users page. You'd have to modify core code, which we do not support. We'll be redoing these older pages in CB 3.x and it'll be easier then to override the template to display whatever you like unfortunately I do not have a solution for now beyond one that'd combine a userlist with CB Auto Actions to allow approval via URL so you could use a userlist as a custom approval page.
