Am not able to duplicate, possibly related to the name of the file, title, or description text? Use a file named say "test" (without double quotes of course). Title as "test" and description as "123". See if the picture uploads without error.

Does this happen on default joomla template?

As Krileon stated previously, try with a default Joomla template. If you're still having problems, give all information (see my signature).
