My Plans show at registration and I need the user to have the following plan options, but I'm struggling with setting this up right... any assistance is appreciated!

1) Monthly Plan at $97 + a One-time setup fee of $199

Question: How do I setup a one-time fee that must be included by still allow the month-to-month fee to be recurring, AND include the first month to be paid with the initial cost of plan $97 + $199?

2) Annual Plan - No setup Fee
(this plan is already establish and OK)
