Session expire allert shows

I also encounter this issue especially on Lost Username/Password Request.

My config:
Joomla 2.5.14
PHP 5.3.26
CB 1.9

CB_Antispoof = OFF
System Cache = Disabled
System SEF = Disabled

BTW, I also tried to unpublish CB Captcha based on what I read from some articles, but still same issue.
