I am also in the same boat... prerequisites show as green but I get no image or audio...

You are not in the same boat - no where near
You are still using CB 1.1 which I assumed your were not using since you posted in the CB 1.2 RC3 area.
Your issue was that database session problem which must be set to none for CB 1.1 and captcha to work (known issue).
With CB 1.2 RC3 and above this is fixed - please upgrade and verify.

After installing, I'm now stuck in "Image with security code embedded in it" and no audio hell ... 
Using an IIS Server, although everything is green with regard to the prerequisites.
Just trying to work out how to do the chmod thing in IIS and see if it helps, but strangely, the Captcha image shows up in the DTRegister event registration form ... I'm confused!
