The link at the top of the plan edit function takes me to the same screen. I get a heading for "Subscriptions" but nothing else. That's the same thing my menu item does. The link for my menu item is below:

Have reviewed your install and the URLs appear to be working perfectly fine if you're not logged in. They will not work if you're logged in, because you do not allow upgrades to them. Notice under "Allow upgrade to this" column in CBSubs > Plans there is a red checkmark. This is due to "Allow upgrade to this:" on the Workflows tab being set to "No".
