How do I Customize Output of fields on a tab.

I want a tab that has three columns of information. This will be presented in a table format. The first column is just a description of the data. The second is a field where the user inputs their own local information. The third is a US average field that the Administrator will update. You can refer to the image attached for clarification. Is there a tutorial on how to do this or is it even possible? I know how to using CB Profile Pro but I want the information in a tab... not on the top.

Best approach is to create an integration to display what you're wanting from a database that you supply values to. Alternatively you could display fields using a Delimiter field by setting title to _UE_BLANK and define it as a blank string in language files then set it to display on 2 lines. Once done format the HTML however way you want it and use substitutions to display field data. Please see below tutorial regarding substitution usage.
