How can I add fields to the Groups Tab? I want subscribers to be able to fill in a form to create a group.

You can't, the groups tab on profiles just shows what categories they own, groups they own, groups they joined, and invites they sent. GJ it self also doesn't support custom inputs for category and group configuration (it's on the roadmap to implement). A form must be completed to begin with to create a group however supplying name, description, etc.. please ensure you're using GJs frontend url as generated from GJs backend found at CB > Plugin Management > CB GroupJive.
