We will be introducing Family Plans to be offered through affiliates who will get a portion of the subscription fee. 
Each family pack, for example, could have 5 accounts, with ability to purchase packs of 5 to add more subscribers to the pack.
We plan to use a commission-based pricing structure. 
  1. Can we use a unique referral code per affiliate and track the amount payable at the end of the year?
  2. My understanding is that Family subscription plan (several subscriptions) and a User Group (group of users) are distinct. Is this correct?
  3. Assuming #2 is true, we want to allow only one Group per Family Plan. Lets call it 'Family Group'. Is this possible?
  4. Will owner have ability to connect with other family groups (whole group), so we could have interconnected families?
Please advise, and suggest how we can best implement this model.

The way CBSubs Family Plans work is it lets User A share their subscription to a limited number of other users.

Sharing will send an email to the recipient users email address for them to accept it. How many they can share to can be a static number or can be based off a field value. If based off a field value you can have another plan that lets them purchase more family shares for example. Typical usecases for it are to share within a household or a company account to share with employees.

It doesn't have anything to do with usergroups, but if the shared plan gives a usergroup the user it was shared to will also be given the usergroup. As for connections it doesn't establish those either, but it's possible to using CB Auto Actions to connect them. It does provide a table list of all users that the plan was shared to for the owner of the plan and the user with the shared plan will see who shared it to them.

How about the referral code? is it possible to track multiple companies that provision pre-configured plans to their staff?

CBSubs Family Plans doesn't use a referral code to manually enter. It's sent to an email address where it can then be accepted via the accept URL provided in the email. The owner of the plan will have a full list of everyone they've shared the plan with and they can revoke the shared plan at any time.

any other way to identify which company the user signed up from?
somehow we need to relate each signup with a provider.
perhaps, we could have a dropdown list of company codes using custom field, that user selects during signup. we could then extract this from db when needed.
possible?

There's a database table that relates them to the "family member" basically. You can do whatever you like there. So for example a CB Query Field to show on User B's profile that they're a part of User A where User A is the owner of the subscription that was shared to User B.

You could also for example use CB Auto Actions to connect to two users via Connections in CB when a family share plan is accepted since CBSubs Family Plans does provide several triggers for doing this. You've quite a few options here on how you could go about that.
