Ok, I think I see the problem here. The renew button is showing despite the plan having auto recurring payments and a grace period configured. The grace period prevents actual expiration of the subscription, but doesn't prevent renewal. So this seams like a bug on our end confusing customers.

 

That subscription is auto recurring and has an active subscription at Stripe. Plan configuration is same as yours. So the customer did indeed manually renew, but this mislead them into doing so. I will work on a fix as it should suppress the renew button for auto recurring subscription during grace period.

While being fixed a workaround is to set "Can be reactivated if expired or cancelled since more than" to "3 days" so they'd have to wait 3 days after expiring to renew, which confirmed suppresses the renew button. Since you're using strictly auto recurring payments here this should be ok and for now will prevent any further accidents like this.
