Have took another look and could finally duplicate. Your issue has nothing to do with CB, CBSubs, or even your ACL. It's your menu module. You're using a 3rd party menu module and it apparently doesn't work very well. Enable the new module called "Menu Test" which is a Joomla menu module and you'll see all your menu links visible and working. I suggest replacing your current 3rd party Menu module or contacting the developer of it for further support.

Yup, normal Joomla menu shows menu items fine. I suspect your custom module maybe have some sort of built in caching (good idea actually), but isn't caching right or isn't parsing menu items right.
