I think that the slip should include a summary of all the information needed for all kinds of offline payments.

I highly agree. We have a feature ticket in place for implementation to CBSubs 1.2 to address the lack of configuration for the payment slip.

I've also added a bug ticket to address the ability to translate the instruction text as was able to confirm it can not be translated.
