CB Login gives error after CB Captcha install

CB Login gives error after CB Captcha install was created by lordxiagan
Installed CB 1.2.2 on the following Joomla:
Using MySQL 5, Hosted under 1and1
Joomla 1.5.12, fresh install (with new database) with migration script from 1.0.x
Rokbridge phpbb3 (Works fine)
All worked fine until I installed CB Captcha (Just downloaded today). Now when I try to login I get 500 Internal Server error. There was a cookie from my previous login that kept me logged in, but I get the same error when I try to log out.
I can log into the admin backend just fine, and the default login works fine. The CB Login is the only issue. Any suggestions?
