I get a similar problem of counter in my Joomla 1.0.10 Stable (migrated from RC2) + CB 1.0 stable:
- Add picture1 OK counter=1
- Add picture2 OK counter=2
- Add picture3 message says OK but the picture does not appears
- Add picture3 again. Same problem.

I did some more tests, trying to understand my problem.
Removing picture, adding others, etc. I get the problem several time, but not on all pictures. So I try to look around...

I think I found the bug: 'quotes'

Add picture1:
- title: Secret spot near le porge
- comment: One of my favourite place
Upload OK

Add picture1:
- title: It is me
- comment: This is a picture of me
Upload OK

Add picture2:
- title: It's me
- comment: This is a picture of me (with quotes in the title)
Upload KO + Message thumb creation + add photo & size to counter

This both concerns the title and description fields.

Need to use the addslashes() function to backslash quotes into the SQL order. I guess it is into the _pgSave() function of components/com_comprofiler/plugin/user/plug_cbprofilegallery/cb.profilegallery.php script

Tested on my website: Problem solved.
