I have found another case, not fully isolated, but I am working on it. I think it might be related to the interaction of the System Cache Plugin + Capcha.

Here are my observations

I turned off system cache plugin, and the problem went away, as far as I can tell. We are going through some user testing at this point. Will know for sure in a couple of days once they report back with their findings.

Its been 10 days with the system cache plugin turned off and no complaints. (we had a dozen a day, and who knows how many people just gave up).

I am using j1.5.10, so it does not look like that brings a positive change to the equation.

So what shall we do to further isolate the problem. I would love to turn the cache back on. Marching orders from Beat?

yes. i was having an issue with my (custom) session variables getting blanked by cb. the htaccess solution seemed to work
